I think the more intellectual items in the list are superimposed over the basic fetish. Fetishes develop at an early age, long before the 'sex between a man and a woman' is even mentioned to you in a typical Christian upbringing. I don't think humans have any capacity at all to develop gripping fetishes based on the moral intellectualizations of life after age 8. The 'fear of sex' is probably secondary, related to the creeping-out that we usually get from kinds of sex that run contrary to our natures. If I'd thought at age 20 that someone was going to make me have straight sex, I would have had a fear of sex too. Bleah. Most likely if he were to go to Canada, buy a case of pork and beans, advertise on xxxxlist and find a legal 16 yo to implement his fantasy scene, any fears involved would be quite different from the 'fear of sex' mentioned. You have to remember that there are literally millions of people who want to be tied up who haven't experienced significant religious sexual repression. It's one of the most common variants on sexuality. There are vast numbers of gay leather bars, several in every big city; read sexual adverts and you'll see gazillions asking for this sort of thing.
