I haven't read the book (probably should), but based on what was portrayed in the movie, I always assumed it was a new experience for Jeroen and so he might be a bit apprehensive, not knowing what it's going to be like. Also, I didn't see the pain as something other than realism. If Jeroen had asked Walt to stop and he didn't I'd see it differently. Not all physically painful experiences are negative experiences and within the context of the surrounding scenes I never thought the intention was to portray it as nonconsensual.
