(note here that I am mostly familiar with the transfeminine side of the question) That about tracks from my experience in trans communities. Cost and also waiting times are definitely an important factor for that low number but even if it wasn't, there's other issues for a lot of trans people have with it: pain, the fact it's not yet a perfect surgery, identity, a lack of dysphoria related to those specific parts, etc
