One has to bear in mind that Jodorowsky's films are full of symbolist or allegorical elements. To ask why the boy is naked is like asking why Rapunzel doesn't get a hair cut. :)

I love El topo, and Jodorowsky's work in general. I would also recommend The Holy Mountain - which has quite a number of naked boys in it, as it so happens. At the beginning, there's the band of boys whose genitals are painted green for some inexplicable reason, and later on there are naked boys who are brought up to hate Peruvians so they can fight insurgents. Another great film is The Dance of Reality - no nudity, but just a great and unique film.
